Summary of Duties

Maintains and enhances referral source relationships. Represents and promotes the Agency by pro-actively calling on referral sources promoting high standards of customer service. Receives referrals and provides information on Agency programs. Processes and distributes referral and reimbursement information to appropriate Agency personnel.

Maintains pro-active and effective communication with Clinical Directors, Clinical Managers, Clinical Coordinators, and Clinical Case Managers as appropriate.

Qualifications

Education & Training: Graduate of an approved school of practical/vocational nursing. Graduate of a National League of Nursing accredited program with a diploma or Associate’s Degree in Nursing preferred. Bachelor of Science Degree in Nursing desired.

License/Certification: Connecticut LPN or RN license, or eligible for RN license in CT, or current temporary RN permit preferred.

Experience: 2 years of clinical experience in acute care setting. Experience in Home Health Care, or Public Health Nursing preferred. Intake and customer relations background is desired.

Skills: Knowledgeable of the principles and practice of Community Health Nursing. Demonstrated excellence in nursing practice. This position requires high energy level, customer relations’ savvy, strong analytical capacity, well-developed interpersonal skills, tactfulness, professionalism, and solid reasoning abilities. He/she must be results oriented. Ability to establish credibility and be decisive. 

Required to maintain a current working knowledge of Federal and State licensure/certification regulations and standards, CHAP accreditation standards, and third party health care regulations, requirements and standards. Knowledge of community resources and reimbursement regulations. Computer literate. Knowledge of office equipment, fax, copy machines, and telephone usage.

Valid motor vehicle operators license, reliable personal automobile during business hours, and active automobile insurance coverage in accordance with Agency policy.

Must be available to rotate weekends, holidays, and after-hour coverage as required to meet patient care needs.
